Intensive Care Coordinator

Job Purpose:

The Intensive Care Coordinator will be instrumental in overseeing the care coordination for clients with a primary substance abuse diagnosis, and other co-occurring mental health and/or homeless issues.  Services will focus on meeting client needs from a holistic, integrated strengths based model including comprehensive assessment, service planning and care coordination which is designed to reduce risks to safety and improve overall health of the client.

BUILDS AND UTILIZES AN INTEGRATED SERVICE DELIVERY MODEL OF COMMUNITY RESOURCES 

By connecting ICC clients to needed services within the community and continued care coordination to address system barriers that are often present for this population; obtaining safe sober housing; accessing mental health services including medication     management and case management; connecting clients to substance abuse services such as ROSC, formalized treatment programs, community support groups, and/or recovery coaches; addressing physical health issues that result from chronic substance use, such as high blood pressure, diabetes and/or traumatic brain injury, etc.; evaluating clients eligibility for, or assisting in the application for state and federal benefits.  

MEETS PROJECTED BENCHMARKS ESTABLISHED IN ICM GRANT CONTRACT 

By attaining regional specific %’s of ICC clients that are:

Avoiding inappropriate admissions to a state hospital after the initiation of ICC services

Engaging in substance abuse and/or mental health services 

Obtaining transitional or permanent housing

Accessing primary medical care for physical health issues

Seeking earned income

By completing a comprehensive strengths based assessment; implementing service planning and addressing multiple aspects of a client’s life; collaborating with the client and significant others to establish a framework of action to assist the client in achieving specific goals; facilitating movement of the client through the continuum of care

APPLIES A MULTI-DISCIPLINARY APPROACH TO CLIENT CARE 

By participating as a key member of interdisciplinary teams; facilitating early intervention and reduction of barriers in order to increase the client’s opportunity for success in substance abuse treatment services and self sufficiency; following up on collateral resources; staffing with appropriate supervisor to determine recommendations; facilitating interdisciplinary team treatment planning and client progress review while advocating for client needs 

PROVIDES INITIAL ALCOHOL AND DRUG ASSESSMENT, SCREENING, DIAGNOSIS AND PLACEMENT SERVICES 

By functioning as an integral part of a behavioral health care system; administering the     KCPCSI, and other screening tools to determine a chemical    dependency diagnostic     impression; rating the severity of alcohol/drug use; recommending an appropriate     placement at the safest and most clinically appropriate level of care in a treatment     setting that matches the client needs; and identifying clients in need of case     management services

USES AGE APPROPRIATE DIAGNOSTIC TOOLS 

By using age specific criteria based on developmental stages when assessing, planning and providing care; developing a knowledge base and expertise in signs and symptoms of addiction, family systems, interview techniques, DSM-IV diagnostic criteria, testing instruments (specifically the SASSI series), resources available in the continuum of care, intervention techniques, American Society of Addiction Medicine Patient Placement Criteria, Kansas Client Placement Criteria.  The age(s) of the clients served range from adolescent to geriatric

MEETS ACCESS TO CARE STANDARDS 

By maintaining adequate coverage for daily operations and on call coverage; conducting assessments as scheduled; being available and willing to travel and respond to identified areas of need; meeting client needs as part of the on-call and standby rotation of clinicians; and ensuring assessments are completed within required timeframes

PROVIDES ACCURATE/TIMELY DOCUMENTATION FOR CLINICAL OUTCOMES

By documenting all client and systems contact in monitoring notes  and case management tracking tools for outcome and productivity purposes; completing an import/export process at least once a day to ensure accurate data is available in the central database in a timely manner; preparing all reports necessary to communicate client status and progress on goals

PARTICIPATES IN QUALITY IMPROVEMENT

By following policies and procedures for gathering and recording data about service delivery outcomes and responding quickly to requests for follow up information as identified by QI specialist

MAINTAINS PROFESSIONAL AND CLINICAL KNOWLEDGE OF THE FIELD

By demonstrating a knowledge base in legal, ethical and confidentiality rules; attending training as needed to obtain CEU credits required for credentialing; reviewing professional publications and maintaining professional certifications and licenses as required

 REPRESENTS THE ORGANIZATION TO THE BROADER CLINICAL COMMUNITY

By establishing and maintaining working relationships and networks with providers and other community agents that increase the range of resources available to the organization; representing the Agency in a professional manner; understanding that personal actions reflect on the Agency and behaving in a manner to meet the code of conduct requirements

Qualifications: 

Masters or Bachelor level preparation with experience in social work, health care field, and behavioral health or substance abuse systems is preferred.

Licensure through the Behavioral Sciences Regulatory Board of KS. 

• LAC – Licensed Addictions Counselor
• LMAC – Licensed Masters Addictions Counselor
• LCAC – Licensed Clinical Addictions Counselor
• LMFT - Licensed Marriage & Family Therapist
• LMLP - Licensed Masters Level Psychologist
• LMSW - Licensed Masters Social Worker
• LPC - Licensed Professional Counselor 

Considerations will be given to candidates with extensive experience working with diverse communities around substance misuse and/or behavioral health. 

Experience and ability to use multiple computer applications including, but not limited to:

• Microsoft 365 Office Suite including Word, Excel and Outlook
• Ability to document client progress in electronic medical record
• Internet/Cloud based applications and platforms

A valid driver's license, dependable transportation and proof of auto insurance must be maintained. 

Ability to lift 25 lbs on a sporadic basis is beneficial.
